It obviously won't be blurred out when you receive the actual item. Seldom seen piece baseball /Field of Dreams history from one of the most memorable film scenes ever (The last shot of Field of Dreams showing all the cars arriving at the farm/ ball field).

Now you can add this rare item to your collection. This is an ORIGINAL one-page Registration form (Measures roughly 8 1/2 x 11 and single sided) to participate in the famous last shot of Field of Dreams. You may have seen the last shot tickets before and the I was in the last shot buttons, but you likely have never seen one of these registration forms from Field of Dreams. This one is unused, not filled out, not cut, with only slight creasing. Comes with a handwritten letter of authenticity from a crew member who worked on the film.

These were handed out to register people to participate in the last shot of the movie Field of Dreams as drivers who would be heading to the farm/baseball field in the final scene of the movie. The production needed people to drive cars in the famous last shot. And in order to get a Last Shot ticket which allowed you to drive in the film, you needed to first fill out this registration form for the scene and picnic. This shows Schedule of events August 3 1988 with a rain date of August 4 1988. A free "Last shot" button.

At the bottom of the form is instructions to send this registration form with a SASE to The Last Shot Shoeless Joe Productions 3430 Dodge Street Dubuque Iowa 52001. That was the production office headquarters for the film Field of Dreams in Iowa during filming.
